The first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to Fairport Family Dental and our caring staff. We encourage questions and do our best to always deliver quality care.

Please take a moment prior to your scheduled appointment to fill in our patient forms. We ask that you complete the forms and submit them through the website before your appointment so we may better assist you.  As of November 1, 2009 we are now required by the FTC (Federal Trade Comission) to see a valid photo ID on all new patients to help prevent identity theft.  This is known as the "Red Flag" rule.   Thank you for your confidence in our office, we look forward to assisting you with all your dental needs.
